Job Qualifications
APN license in New Jersey
CDS/DEA license
CPR certification
Minimum of 3 years experience as APN in geriatrics, pain management, oncology, or palliative care
CHPN ACHPN or Pain certification preferred
Team Player
Excellent interpersonal & communication skills
Willingness to travel within NJ
Expert assessment, diagnosis, and treatment skills
Must be self directed, self motivated, and organized
Reliable transportation
Job Responsibilities
Palliative care consultation in LTC and AL facilities
Development of care plans to meet patient and family physical &psychosocial needs
Skilled and aggressive symptom management
Prepare families for transition to hospice
Education of HCP about palliative care
Role model and mentor to RN and MD and all levels of training
Work in collaboration with on site MD
Thoroughly documents on consult form and progress notes
Accurately completes billing information in a timely manner
